In addition to the services of Western Reserve Hospital, physicians that provide hospital based care within Western Reserve Hospital are covered by this HFA policy and include:

Radiology Services: Northern Summit Radiology, LLC / Akron Radiology Inc. / WRH Physicians, Inc. (WRHPI)

Emergency Room Services: Summa Emergency Associates / Academic & Community Emergency Specialists,
Inc.

Hospitalists: Unity Health Network (Hospitalist group providing inpatient daily care services)
Western Reserve Hospital Physicians, Inc. (WRHPI) (primary care and specialty care physicians employed bythe subsidiary of Western Reserve Hospital)

Services provided by all other physicians not listed above are NOT covered under the
Western Reserve Hospital Financial Assistance Program.
