On the day of your discharge, your physician must write an order for the discharge before you can leave the hospital. It is very important that the physician has all of your medical information (test results, etc.) before you leave the hospital. Your medical care team and nursing team will be sure you have all of your medication and home care instructions completed prior to your discharge. In order to make your discharge as simple as possible, our New Choice Pharmacy can fill and deliver your home going prescriptions right to your bedside. One of our pharmacists will also visit to advise you on any medications you receive, and answer any questions you might have. Please speak to your physician or nurse if you would like to take advantage of this service. Be sure to identify a designated family member or friend that can transport you home upon discharge. Patients are responsible for arranging their own transportation home from the hospital.
